Every new patient will need to complete an online registration process PRIOR to your visit with Dr. Andersen.

The online process includes pre-registration forms as well as completing an online health questionnaire. This helps your visit to the office go smoothly so that we can remain on time for each patient.

Please note our office must provide you with the login information. Once you schedule your appointment, you will receive login information, and at that point, you can access the portal.

Plan ahead, as this process can take anywhere from 30 minutes to an hour.

If you have any trouble completing the online process please feel free to call us at 925-400-6900. If you would like to complete this process in the office prior to your visit please arrive no less than 1 HOUR before your scheduled appointment time.

Please remember to ALWAYS hit “SAVE” after entering in any new information online!
